Who We Are
Transitional Steps is a community-based nonprofit organization dedicated to supporting individuals and families navigating major life transitions across the lifespan. From youth entering adulthood to veterans reintegrating into civilian life, seniors adjusting to new levels of care, and families experiencing birth or loss, we exist to address the critical gaps that often emerge during periods of significant change.
Life transitions can disrupt identity, stability, and access to essential systems of care. Transitional Steps responds by providing structured, relationship-based programming designed to strengthen resilience, promote independence, and reconnect individuals to community resources and support networks.
Our services provide coordinated support and include:

What sets Transitional Steps apart is our integration of lived experience, professional expertise, and data-informed practice. Our multidisciplinary leadership team brings experience in health sciences, clinical mental health training, philanthropic program management, data analytics, and community-based outreach. This allows us to design programs that are responsive, measurable, and adaptable to the evolving needs of individuals navigating critical life transitions.
Through strategic partnerships and continuous learning, we walk alongside those we serve to strengthen stability, increase access to care, and support long-term resilience across changing seasons of life.
